JOB SUMMARY
Position: Outreach Services Case Manager

Job salary: $26 - $30 an hour

Work Performed:
  • Conduct client intakes, assessments, and develop a service plans
  • Arrange client meetings, typically online or over the phone, to review the service plan and coordinate the next steps
  • Delegate cases to Case Workers and Link Workers as appropriate and in consultation with the supervisor
  • Facilitate client access to community resources, including housing, food, clothing, educational programs, vocational opportunities or services, providers to teach life skills, and mental health services.
  • Follow up in a timely manner on client activities and progress relevant to the service plans
  • Advocate for the clients
  • Evaluate the service plan and progress with the client, and amend the plan if deemed necessary by both the client and Case Manager
  • Uphold clients’ autonomy and self-determination and actively involve clients in the decision-making process.
  • Organize and manage client data, files, schedules, dates, and information as per agency protocols
  • Enter accurate and objective case notes about important events and progress in Canfit, the agency’s data software program, throughout the entire working relationship
  • Maintain client privacy and confidentiality
  • Communicate with community agencies or partners and identify additional or alternative services to provide referrals and maximize assistance for clients
  • Maintain current and comprehensive knowledge of community resources through networking and community engagement, professional development
  • Represent JFS in relevant collaborations and committees
  • Other duties as assigned

Qualifications:
  • Diploma or degree in Social Work or related Human Services program (behavioral/social sciences or community services).
  • Minimum of three (3) years of experience in case management or related services that have provided a sound understanding and knowledge of working with clients from diverse cultural and socio-economic backgrounds.
  • Familiarity with current community resources.
  • Cultural sensitivity and/or diversity and inclusion training required
  • A second language is an asset
  • Familiarity/understanding of Jewish culture and the Edmonton Jewish community is an asset
  • Strong organizational and communication skills
  • Familiarity with healthcare and social service systems
