WORK PERFORMED
  • Visits patients and/or family members routinely to obtain and exchange diet-related information to ensure that the food is consistent with assigned nutritional plan and that food meets patient’s preferences and dietary needs. Provides assistance during meals, observes and charts food intake and communicates with the Registered Dietitian as indicated. Provides routine diet instruction as delegated by the Registered Dietitian.
  • Acts as a liaison between Clinical Nutrition and Food Services.
  • Reviews patient’s/resident’s information as required from the medical record and liaises with relevant staff to obtain added necessary information when needed. Charts activities in the patient’s medical record. Enters and processes information using the nutrition computer system.
  • Conducts nutritional screenings and preliminary nutrition assessments to determine malnutrition risk using standardized forms/assessment tools and refers to Registered Dietitian for further evaluation as indicated.
  • Assists in implementing nutrition care plans designed by the Registered Dietitian and provides feedback on patient progress.
  • Sets up nourishments and marks menus as per standard diet writing guidelines.
  • Conducts computerized nutritional analysis of menus, recipes and food records using the nutrition computer system.
  • Reports patient complaints with food quality/service and communicates problems to dietitians, food service supervisor or applicable staff as appropriate.
